In today’s healthcare system, costs keep rising while patient access and satisfaction continue to fall. It’s no wonder both patients and physicians are burning out.
We’ve chosen a different path.
Instead of billing insurance for every visit and procedure, we offer a simple, flat membership fee—monthly or annually—that gives you full access to your physician. No copays, no surprise bills, and no waiting weeks for an appointment.
This model works because it puts time and trust back into the doctor-patient relationship. Patients in practices like ours experience better outcomes, fewer ER visits and hospitalizations, and more peace of mind knowing their doctor is just a call, text, or visit away.
Most doctors are stretched thin, managing thousands of patients with just 15 minutes per visit. At Smith Family Medicine, we keep our panel small—under 200 patients—so we can spend real time with you. Most visits last an hour.
Our longer, on time, appointments can happen in the office, by phone, via video, or even at home when appropriate.

A modern model of medicine rooted in an old idea: your doctor works directly for you, not an insurance company.
While we require members to still carry insurance, we don't bill insurance. Your membership fee covers almost all your care with us.
We accept patient with Medicare and are required by law to bill Medicare for services rendered. However the membership fee is still required.
By law, we can not see patients with a Medicare Advantage Plan (also known as a Medicare HMO Plan)
Yes you can use your HSA or FSA account to pay for membership fees.
Your membership fee covers your care with us. However you are still required to have insurnace.
We suggest choosing a High deductible PPO plan, as they have lower monthly premiums. However we can still see patients with PPO, EPO, HMO, Medicare and even Kaiser Plans. By law we can NOT accept Medicare Advantage patients.
Additional things to consider when choosing a plan:
- In network (vs out-of-network) specialist (if you have or will need one in the future)
- Prescription drug coverage and pharmacy of choice
- In network hospitals
We coordinate referrals, help navigate systems, and follow up to make sure nothing falls through the cracks.
No, membership fees do not get applied to your insurance deductible. But you can use your FSH or HSA account.
Labs are not included in your membership fee, however we collect labs in our office free of charge and have a heavily discounted cash pay price with Quest. We can also submit them to a lab of your choice to be billed to your insurance if you choose.
We can order imaging at any of your preferred local vendors. In most cases we are also able to submit these for insurance coverage.
While we strive to provide care as soon as possible to our patients, it is important to remember that we too have families and need to maintain a healthy work-life balance. We will always prioritize and be available for emergencies.
Our easy to email platform allows our patients to send us a message when convenient for non-emergencies such as medication refills, lab requests/questions, appointment requests, etc. This allows our physicians the flexibility to assess the need and determine appropriate timing of response which is either same day or next business day.
